In our Hamburger Straße housing and assistance offer in Bad Oldesloe, people with different individual assistance needs live. We as klaarnoord gGmbH provide participation services for people with mental disabilities and for people with mental disabilities and mental illnesses. In our multi-professional team, we support and advise our clients in implementing their personal goals in order to achieve a great deal of participation. We think of the human being and provide our services individually and socially oriented.

Overview of tasks

• Person-centred individual case aid and reference work
• Assistance in the activities of daily life
• Guided tours, walks and the development of the social space
• Official contactin and companionin moments of crisis
• Development of offers in the house, in the neighborhood and in the region
• Participation in the maintenance, activation and expansion of the participation structures of our clients
• Creating social history reports, assistance planning and documentation
• Participation in service meetings, case meetings and commissions
• Communication at different levels with clients, candidates and legal supervisors
