Individual (psycho-) social and social counselling of patients: Support in the development of new perspectives after hospital stay Cooperation and participation in multi-professional teams Coordination of rehabilitation and aftercare services Cooperation with non-Clinical institutions, institutions of rehabilitation and authorities Networking in the Social Psychiatry of Hamburg Documentation and performance detection

Complete study of social education or social work with state recognition Experience in the psychosocial field, ideally clinical or health-related Consultancy, resource orientation and responsibility Competence with regard to social pedagogical interventions also in crisis and emergency situations, intercultural competences and knowledge in social legislation, or the willingness to work on new topics We are looking forward to applicants: those who have undergone special training and bring this knowledge
