Pilot in the model project QplusAlter – senior citizens, care for the elderly, counselling, care, neighbourhood

You advise and accompany older people with support needs and (careful) relatives, especially people with difficult access to the help system, in the process of formulating their own interests and finding the appropriate support for everyday life. To do this, explore all available resources and include them: the capabilities of the human being, technical support, family, friends and neighbours, networks in the quarter, and professional services. As part of strategic networking work, you will explore and develop knowledge, contacts and resources in the district, which can be accessed in (later) consulting and accompaniment processes and thus contribute to the further development of quarter structures. You place the self-reality of man, his self-responsibility and decision at the heart of your action. And you don't do that alone! In our interdisciplinary project group we perceive and promote the personal potential of all our colleagues. Do you have a training and experience as a nursing specialist or health caregiverin, a training as a social education teacherin/social workerin, nursing managerin or Gerontologin, are employees with comparable qualifications or cross-comersin with experience in nursing and health care?
