Collection of a social anomalies to detect the psychosocial situation of our patients Recognize and process personal and social problems related to illness or disability Consultation, mediation and organisation of specific assistance programmes for the preparation of dismissal and security of care, e.g.: organisation of medical, professional and social rehabilitation measures Coordination of outpatient services such as domestic nursing, SAPV, food on wheels and provision of healing and aids mediation of semi- or fully-stationary care facilities Consultation in matters of care law and the initiation of care procedures in accordance with §§ 1896 and 1821 BGB Support in the enforcement of social and financial claims

Complete study of social education Embossed consulting skills, empathy and joy in dealing with patients in partially stressful situations of life Very good knowledge of social law and health and social affairs Safe handling of common computer and readiness to be incorporated into digital documentation systems Very good knowledge of German in word and writing (at least C2 level) Ideally experience in the social service of a hospital
