Care Advisor Hanover (m/f/d)

In the care consultancy you are the interface between clients, relatives and specialist areas. You conduct consultations, analyze needs and implement suitable solutions. On average, you will accompany around 90 consulting appointments per month, about 60 to 90 minutes including documentation.

Framework conditions: Location: Hanover, Hahne Holding Central Areas. Hours: part-time or full-time with 20 to 40 hours per week. Working hours: Flexible appointment with clients and relatives; independent organisation of consultancy talks. Meetings: One to two management meetings per week between 9:00 and 15:00. Mobile Work & Cars: At least 50% mobile work possible; Cars are placed, use with own shares. Type of contract: Unlimited.

In the case of a weekly working time of 40 hours, the monthly gross salary is between € 4,200,36 and € 4,365,66, depending on the experience.

Your tasks: Demand analysis: Systematic detection of individual support needs taking into account the findings of an assessment by the medical service. Care counselling: implementation, documentation and logging of the consultations pursuant to Section 37(3) of the Elftes Book Social Code. Performance consulting: Reconnaissance on the budgets of the nursing company and the performance portfolio of Hahne Holding. Documentation: Structured preparation and maintenance of the average 90 consultations per month. Interface work: forwarding customer potentials and relevant information to internal areas of expertise.

Your profile: Qualification: Complete training for nursing staff. Experience: practical experience in consultations according to § 37(3) of the Social Code Elfte's book. Mobility: Valid EU licence Class B is required. Operation: Self-employed, structured and well organised. Communication: Consultative, solution-oriented and binding in appearance.
