for our **Sozialstation Nord/West,** Albert-Schweitzer-Straße 52c, in 09116 Chemnitz and Sozialstation Ost, Max-Saupe-Straße 43, in 09131 Chemnitz.

What we want from you:

• Senior Consultant (m/w/d) or Social Assistant (m/w/d)
• Experience in outpatient care of the elderly is of advantage, but no need – more important are motivation and heart for the profession
• Flexible, ready for use and reliable in turbulent situations
• Self-employed working with a good look for the essential – at the same time a real team player
• Embossed customer and service orientation as well as an esteeming, empathic approach to people
• driving licence class B and safe driving practice – joy in mobile work included

Your task with us:

• They take care and care in the home environment independently and ensure the well-being of the clients.
• They reliably implement the care order, support the team in everyday life and work closely with professionals.
• They carry out the nursing process, document carefully and are in a valuable exchange with relatives as well as cooperation partners.
