Tasks:

• advice and coordination of connection services, such as outpatient/stationary rehabilitation measures, domestic care, care aids, short-term and long-term care, hospices and support services in everyday life
• Support for social and psychosocial issues – incl. First consultation, assistance for disease processing and crisis intervention
• Organization and supervision of individual supply models – with the aim of maximising patients and relatives
• Interdisciplinary cooperation with medical, nursing and therapeutic teams as well as possibly Further transfer to external contact points

Requirements:

• Complete study of social work / social education (Diplom, B.A. or M.A.) Professional experience in health
• Finding knowledge in social law as well as dealing with rehabilitation, care and support offers
• Safe, empathic appearance in the advice of patients and relatives – even in challenging situations
• Organisational skills, independent working and pleasure in interdisciplinary cooperation
