You conduct care consultations according to § 37 para. 3 SGB XI. In doing so, you ensure the quality of home care, identify the individual wishes, needs and resources of our customers (m/w/d) and develop suitable support measures together You advise customers (m/w/d) as well as their relatives on nursing, care and relieved support offerings and inform them about appropriate care insurance services You take care of the holistic care and care of people in need of help in their home environment according to SGB XI and make sure that You feel good all around As part of SGB V treatment care, you carry out medical-care measures independently and ensure high-quality care You are a central contact person for our customers (m/w/d), their relatives and supervisors (m/w/d) – with an open, value-assured communication and advice on your side As part of our multi-professional team, you actively contribute your expertise and work closely with colleagues (m/w/d) from various departments You regularly take part in team and service meetings, service assignments and case meetings and thus shape the further development of our care processes with
