Your future job: For our prestigious partner in the outpatient care service, we are looking for you as a dedicated care specialist (m/w/d) as part of the employee transfer in Sömmerda and the surrounding area. They take over the professional care of patients in their home environment and contribute to an individual, value-adding care.

Your tasks:

• Treatment care: implementation of medically prescribed measures (medicine, association change, vitality control, compression therapy)
• Basic care: Support in body care, mobilization, nutrition and storage
• Care planning: Demand collection, preparation of individual care plans, professional documentation (Medifox)
• Advice: guidance and support of patients and relatives
• Coordination: Cooperation with doctors, therapists and other service providers
• Quality assurance: compliance with nursing standards, hygiene requirements and legal requirements
• Psychosocial support: promoting self-employment and support in challenging situations
• Service car agreement possible

Your profile:

• Senior Consultant (m/w/d)
• Engaged personality with good knowledge in the outpatient care of older people
• Willingness to shift service (Mo-Fr: change early and late shift, night shift/readiness to leave)
• Responsibility, empathy and independent working
• Class B driving licence mandatory
• Good knowledge of German in word and writing
