The Hospital Maria Hilf Warstein is a specialist clinic for geriatric and internal medicine with 113 flatbeds and ensures the medical care of patients in Warstein and the surrounding region.

Our house is divided into the Departments of Internal Medicine with the focus on cardiology and gastroenterology as well as geriatrics. The medical offer is supplemented by the function departments anesthesia and radiology, which support modern diagnostics and treatment. In addition, our house has a central emergency intake (ZNA) for acute care and a department for intensive care for critically ill patients.

The medical care center (MVZ) Warstein is also connected to the hospital, which ensures outpatient medical care and strengthens the cross-sector care of our patients.

• Advice and support of patients and their relatives in social, psychosocial and social matters

• Organisation and coordination of post-stationary care in the context of legal release management

• Intermediate treatments, rehabilitations, short-term care, stationary care facilities and outpatient care services

• Advice on the benefits of sickness, nursing, pension and social security benefits and other social benefits

• Close interdisciplinary cooperation with doctors, nurses, therapists and external cooperation partners

• Support for the application of social security benefits

• Documentation of consulting and coordination processes in the hospital information system

• Participation in the continuous development of redundancy management and cross-sector cooperation

• Successfully completed studies of social work (B.A./M.A.) or social education (Diplom/B.A./M.A.), ideally with state recognition

• Professional experience in hospital, healthcare or social services is desirable

• Good knowledge of social law, in particular in the fields of SGB V, SGB IX and SGB XI, are advantageous

• Representative communication and consulting skills

• Empathy, sense of responsibility and an appreciation of patients
