Due to chronic illnesses, need for care and a shortened hospital stay, domestic settings are increasingly addressing questions about care situations, further care and a possible life change. As part of the familial care, individual counselling and training is accompanied by the transition of the stationary hospital stay to the haemorrhage. Families play a central role in the care and care of their relatives and are supported with individual care training and group courses in this role and task.

This is what awaits you:

• Contacting patients, patients and their relatives, as well as consulting and training for this to design the home care situation in the context of the concept Familiale Pflege

• Conducting individual care training with patients, patients and their relatives, both during the stationary stay and in the haemorrhage after dismissal

• Advice on the subject of patient provision and care law and takeover of lectures and representative tasks

• Interprofessional cooperation in the UKSH and with external partners, including dementia experts (LOGGiA), care bases and other networks

• Support for cost accounting and quality assurance in relation to cost providers and within the UKSH

Take this with you:

• A completed three-year training as a nursing specialist and first experience in outpatient care and on social law topics

• Qualified training or a long-term professional experience in guidance, education and advice, or further training in case management

• A high degree of independence, creativity and solution orientation taking into account the individual family problems

• Safe handling of MS Office (Word and Excel) and Agfa ORBIS

• Class B driving licence
