Your tasks

• Planning, coordination and implementation of support measures to help customers and their families to manage in difficult times and overcome dependencies.
• Surveying users of the service to assess their current state, needs, strengths and weaknesses.
• holistic consideration of each case and definition of targets tailored to it.
• identify the needs of clients with regard to their individual financial, emotional and physical circumstances.
• Creating an individual care plan that includes regular visits, access to support and resources, as well as clear goals for improving living conditions.

You bring

• A completed degree Diplom-Socialarbeiter:in or Diplom-Socialpädagog:in
• or Bachelor or Master's degree or a more comparable qualification
• A high motivation
• A high level of initiative, empathy and flexibility
