• Social and psychosocial consultation of patients and their relatives
• Participation in anamnese and findings collection (social amnese and psychosocial diagnosis) and therapy planning
• Advising and requesting further measures in the context of the discharge management, e.g. outpatient and outpatient care, medical rehabilitation services, benefits for participation in working life, integration aid, reintegration into working life
• House and neighborhood visits, accompaniment to public authorities, if necessary. Related articles, use of your own car (a distance package of 0.30 €/km is paid)
• Separation of entitlements to benefits as well as financial assistance for the maintenance of livelihood (e.g. sick leave, basic insurance, housing allowance, civil allowance)
• Planning and implementation of the Federal Dispute Act
• Consultation according to SGB IX
• Advice on the right of care including care, precautionary and patient provision
• interdisciplinary case work, participation in therapy meetings, station meetings, supervisions etc.
• Participation in internal and external training
• Direction and participation in social pedagogical and social therapeutic groups such as table tennis and cooking groups and family groups
• Close cooperation and coordination with the social service at the main house in Weimar
• Cooperation with authorities and institutions outside the clinic, in particular with the various social service providers, regular correspondence, representation of the clinic in the institutions of the community psychiatry association

IHR PROFIL:

• completed (special) university studies, subject of social work/social pedagogy (Diplom/Bachelor/Master)
• Knowledge in all areas of Social Codes (SGB I to SGB XII) and experience in application
• Experiences in dealing with hospital information system (e.g. ORBIS) and MS-Office products
• Willingness to continue and further education
• Loadability, self-motivation and enforcement capability
• independent, targeted, self-responsible as well as economic thinking and working methods
