The Entlassmanagement/Sozialdienst department is organisationally assigned to the 5 Medical Management division of the Medical Director and Chief Executive Officer. The department consists of the two occupational groups of social workers and case managers. The common concern is the precise organisation of post-stationary care for the clinics and patients. The employees in the dismissal management/social service work in teams in the respective clinics or centers. Thus, the expertise of social work and care competence can be provided and optimized for all areas of the clinic and patient and relative counselling.

Supporting individual teams in the release management/social service Organisational and administrative support of departments Direct personal contact with patients and relatives Telephone and written correspondence with patients, relatives, co-chairs, internal and external cooperation partners General office and assistance activities at usual office times Form Care & Digitization Support for the introduction of new employees

Senior Consultant (m/w/d) Self-employment, reliability and resilience, self-initiative in thinking and trading Safe handling of Microsoft Office applications Organizational skill Reliability, sense of responsibility High communication skills and ability to work on a team Friendly, binding and safe handling in personal and telephone contact with patients, patients and relatives
