Nursing case support (m/f/d)

We are looking for nursing case companions (m/w/d) for the following tasks: Assessment and care Early clarification of patient's post-care needs Pre-screening and detailed documentation of the patient situation Objective and assistance planning Create and maintain release plans Maintenance of EM screenings and checklists Coordination and organisation Organisation and coordination of the care process Exchange with medical and therapeutic staff via treatment plan and release dates Monitoring and evaluation Monitoring and adaptation of measures Evaluation and quality control, including follow-up in further supply

Completed training in nursing or relevant university degree Further training in case management or readiness for this Representative consulting skills, communication and team skills Good coordination and organisational capacity Responsive, solution-oriented and structured operation A certain degree of enforcement and resilience
