Your New Task You take over the continuous, person-centric accompaniment of adult people in the context of outpatient integration assistance and create sustainable, professional support relationships. Your tasks include: the collection of needs and the participation in assistance planning according to SGB IX the development, implementation and evaluation of individual participation objectives promoting an independent life management support in the organisation and management of life-structural requirements the accompaniment in contact with authorities and medical institutions stabilisation in psychosocial crisis situations through professionally sound intervention Promoting social skills and building and maintaining sustainable relationships careful documentation and cooperation in the multi-professional context You work independently, reflect and methodologically guided. In doing so, you combine professional relationship design with structured, targeted assistance and actively contribute to securing sustainable participation. Your task – meaningful and effective You accompany adult people with essential disabilities in their own living space. These include people with mental illnesses, mental disabilities or impairments from the autism spectrum who want to live independently or live independently and whose support needs can be covered outpatiently. You work in a person-centric, resource-oriented and targeted manner on the basis of integration aid under SGB IX.
