Care advice for the care support point (m/f/d)

Coordination of the entire range of services for nursing persons and their relatives: You advise independently on all questions of the outpatient and inpatient services for elderly, needy or needy persons and their relatives. You determine individual needs in a structured consultation process. Together you will develop a support and help setting. You support the application and assist in the use of care and further assistance. You advise on social benefits, also beyond the pure nursing advice and the services of the nursing care insurance companies according to SGB XI, as well as for housing adaptation. Networking with Hanauer organisations and groups also active in the elderly and in the health and social systems is also part of your area of responsibility. In addition, you provide public relations. You are a contact person for complaints from the outpatient and inpatient care area. Care management activities: You collect data on existing services and services, analyze them and prepare them. You are organising coordinated cooperation with advisory bodies, volunteers and self-help organisations. You build an efficient networking structure of all assistance, care and care services for local and regional care workers. Case management activities: You analyse the supply needs in complex case design, develop supply targets and help plans. The selection and placement of the assistance, services and facilities required in individual cases is also your responsibility. You initiate the use of the benefit for the first time. You evaluate the supply management and adapt it. You also document the results of the case management.

Complete study of applied nursing sciences, in nursing management or social work or Social education with a focus on care (consultancy), a comparable study or a completed training as a nursing specialist (examined nursing caregiver, healthcare and nursing caregiver, health caregiver and child caregiver) with at least three years of professional experience in nursing advice Additional qualification in case and care management as well as care advice according to the criteria of care insurance (§ 7a SGB XI) Comprehensive knowledge of social legislation, focus on care Experience in public relations, including in social media High commitment, independent, creative and solution-oriented action Representative communication, organisation and coordination capabilities A service-oriented and empathic basic attitude Ability to team and project work Good PC knowledge and safe handling of MS Office applications Ownership of the passenger car license class B and the willingness to use your own car for service purposes (travel cost refund under the Hessian Travel Cost Act)
