Your duties

• Implementation of individual and holistic care in outpatient service
• Planning and optimization of care measures
• Documentation of the respective care processes
• Support for daily challenges in the home Environment of clients
• Contact for members, doctors, colleagues

Our requirements

• Completed training as old and old Health and Senior Consultant (m/w/d)
• Open and sensitive handling of people in need of care and their relatives
• Class B licence
• Detection Masern (if born after 1970)
