Coordination of Outpatient Educational Assistance (m/f/d)

As coordination in the outpatient area, you control and accompany help processes from the request to the end of the help. Your tasks include: technical support, advice and support of the employed pedagogical professionals Examination and coordination of requests by youth offices Case control and ensuring the implementation of the agreed aid plan objectives Preparation, participation and follow-up of help plan discussions close cooperation with youth offices, families, schools, kitas, therapists:inside and other institutions professional and transparent cooperation with young people, families, caregivers, the youth office and all professionals involved in the assistance process and internal contact persons Checking and ensuring technical documentation in the course of assistance Compilation and professional participation in progress, expertise and final reports Organisation and implementation of case meetings Crisis and conflict management in challenging aid procedures Participation in child protection processes according to § 8a SGB VIII Accompanying new recordings, fall handovers and terminations of assistance Cooperation with administration and accounting, in particular on accounting and case-related issues Participation in the further development of processes, concepts and quality standards
