Interdisciplinary case coordination: In close interdisciplinary cooperation with other specialist departments, you actively participate in meetings, patient conferences and department meetings. Competent Social Rights Consulting: You take over social counselling and support all questions on rights, services and claims. Active network management: Networking with care facilities, care institutions, employers and cost-beneficiaries is one of your tasks, as well as the corresponding transfer of patients. Targeted rehabilitation: You coordinate close-to-home transfers, geriatric rehabilitation measures and follow-up treatments and advise on professional rehabilitation, retraining measures and gradual reintegration into working life.

You have a completed study of social work, social education (B.A.) or a comparable qualification; dedicated beginners are also welcome.
