The main tasks of the Social Medical Service for children and young people with disabilities include the care, counselling and support of children, young people and young adults with mental and physical disabilities and their families.

The initiation of targeted assistance or targeted support is carried out in the internal and external services. In addition, opinions and opinions are drawn up in cooperation with the doctor:in the department.

The area of responsibility comprises essentially: 

• Advice for young adults with disabilities to e.g.
• Integration aid
• Help for education
• Care services
• Disabled card
• special educational needs and
• Living, care and work opportunities
• advice on outpatient, partial and full-time assistance for children, young people and young adults with disabilities and their caregivers
• Advice on problems, conflicts and difficult situations of life
• Assessment in the context of the overall and participation plan procedure, federal participation law (BTHG) and aid needs determination – housing (HMB-W)
• Aid plan proceedings under § 36 SGB VIII in cooperation with the Office for Youth, Family and Women, and
• Cooperation with established doctors: inside, hospitals, offices, public and free disabled institutions.

Preparation for an application is:

• a completed study of the Diplom-Social pedagogy/Diplom-Socialarbeit (FH) with state recognition or
• the completion of the Social Work Programme B.A. with state recognition.

We are looking for a personality that has good communicative skills, is resilient, responsibly and team-capable, and is identified with the tasks of public health services within the framework of civil friendliness.

The secure application of the standard software MS Office, willingness to continue and further training as well as the willingness to cooperate constructively with network partners.

Experience in the field of demand determination in the context of the overall plan procedure of the BTHG (e.g. HMB-W, individual treatment and rehabilitation planning and/or BENI), professional experience in work with children and young people with disabilities, as well as knowledge of the legal basis (e.g. SGB VIII, IX) and the possession of a valid driving permit are desirable.
