Social-worker advice and support for stationary and outpatient patients of the somatic departments in social problems with a significant inclusion of the next social reference person Implementation of a re-integrative 'Starthilfe' in the sense of dismissal management in the form of care accepted by patients/clients Support for the continuity of care in the transition between hospital and home care/care home/home Detection of situations where post-care is not guaranteed Crisis intervention interdisciplinary cooperation in dismissal planning Cooperation with non-stationary institutions clarification of questions concerning clients in the home environment

Diploma of an Academy of Social Work or completed studies at a Fachhochschule für Sozialarbeit Responsibility for ownership and dialogue very good knowledge of German in word and writing Willingness to cooperate with extramural services Ability and experience in organizational tasks readiness to flexible service time good skill in human leadership, in particular value-assuring handling of employees and patients as well as their relatives solid knowledge in MS Office (Excel, Word, PowerPoint)
