In the Department of Entlassmanagement/Social Service of the University Hospital of Bonn, the following office is to be filled in full-time (38,5 hours/week) at the earliest possible time:

Tasks

Your tasks:

• Supporting our patients and working with the medical staff during the hospital stay and preparing for dismissal
• Communication of outpatient aids and aids
• Organisation of nursing homes, short-term care places
• The advice and support of our patients on social/social insurance issues
• Interdisciplinary and multi-professional cooperation with all areas of the hospital, especially doctors, nurses etc.
• Cooperation with external partners such as nursing homes, outpatient services, authorities etc.
• Medical connection rehabilitation

Requirements profile

Your profile:

• Pre-experience in the field of clinical social work desirable
• High social competence, empathy and adequate contact skills in dealing with seriously ill people and their relatives
• Organisational skills, communication skills and loadability
• Team skills, professional and interdisciplinary
• Funded social knowledge (e.g. SGB IX, XI, XII and SGB V)
• EDP knowledge
