As a Palliative Care specialist, you take on the independent supply of heavy-duty people in the outpatient and stationary environment in the Hanover region and the surrounding area. You coordinate palliative and nursing care, accompany patients and relatives and work closely with doctors and other cooperation partners.

Framework conditions: Location: Hannover and Umland region. Hours: full-time or part-time, 30 to 40 hours per week. Working hours: Use in early and late services from Monday to Friday. On weekends, a planned call will be made according to the schedule. salary: from 4.475.28 € gross per month at a working time of 40 hours per week. Start: 01.05.2026

Your tasks: Palliative care: Individual, professionally informed care of clients with SAPV regulation with the aim of ensuring quality of life and symptom control. Coordination of care: coordination and organisation of palliative and nursing measures in close cooperation with doctors and cooperation partners. Support for patients and relatives: support in the last stage of life through consultation, orientation and sensitive support. Symptom reduction and stabilization: participation in measures to alleviate pain, anxiety and other stressing symptoms. Your profile: Qualification: Complete training as a nurse and further training in the field of palliative care.
