through the close-to-home and neutral advice of caregivers and their relatives in all questions in the pre- and environment of care through anonymous and personal guidance and decision-making assistance (telephone and personal as well as on home visits) through coordination of health services (case management) due to the care advice acc. § 7a SGB XI through public relations: presentation of the care base at trade fairs and information events, presentation of work in committees with quality assurance and documentation through network and district work by participation in supervision, training and working groups

with your completed studies of social work or social education, each with state recognition, nursing, care management or a comparable qualification as an alternative to your completed training for health and nursing staff (m/w/d) with the additional training Case Manager (m/w/d), your completed training for nursing staff (m/w/d) with the additional training Case Manager (m/w/d) or with a comparable training with your training as a care consultant (m/w/d) according to § 7a SGB XI with your professional experience in the above mentioned area (required) with your training in the field of care, case management or law (required) with your experience in consulting processes (of advantage): communication and discussion Moderation of case conferences Networking of actors and case control Negotiating techniques with social services

with your knowledge in social law (required) with your good computer skills in the application of MS standard software (Word, Excel, Outlook) as well as in the area of documentation of cases in the computer system with your organizational capability, reliability and communication capability with your consulting skills and your experience in the application of documentation programs with your conceptual thinkability and your structured and independent working methodology with your high performance and your conflict management with your team skills with your class B driving licence This position is to be filled in full-time (part-time possible) in the social and nursing consultancy department, care authority in the Social Affairs Office and participation at the Backnang site. 40% indefinite and 65% up to 31.
