You accompany people as part of the outpatient integration aid: You support people with dependency disorders, among others, in the independent management of their daily lives, in administrative matters, in participation in social life and in the perception of social contacts, in crisis interventions You maintain contact with the service providers, authorities and legal caregivers of your clients Implementation of the management planning according to the cost management requirements Administrative tasks associated with the case management such as documentation and file management Participation in team meetings, supervision and training As part of the activation, you also offer group activities
