Your duties

• Planning, implementation and evaluation of individual care measures based on the care process
• Activating and rehabilitative care of geriatric patients with the aim of restoring and maintaining the greatest possible independence
• Promoting mobility, everyday competence and participation in an interprofessional treatment concept
• Close cooperation with doctors, therapists and social services in the geriatric team
• Advice, guidance and training of patients and their relatives with regard to domestic supply
• Ensuring complete and professional care documentation in the hospital information system
• Participation in team meetings, case conferences and training

Your profile

• Senior Caregiver (w/m/d), Senior Caregiver (w/m/d) or a comparable qualification
• Self-employed and self-responsible work in the transferred area
• Motivation to organize and manage the everyday life of a rehabilitation
• Very good communication skills and cooperation with all professional groups as well as friendly, sovereign appearance
• Pleasure of collegial cooperation
• PC identifiers corresponding to the task area
