The association CJD NRW Nord is one of the largest associations within the Christian Youth Village Plant in Germany, with about 1,300 employees and a regional spread from East Westphalia to the Lower Rhine, from Münster to the Rhineland. As a case manager (w/m/d) in the vocational training work, you advise and support young people in professional rehabilitation, with the aim of professional and social participation. In dialogue with those concerned, you control the individual measures of integrative training in the vocational training work, and you also agree in the multi-professional team, with the cost-beneficiary as well as external bodies and reference persons of the persons concerned. Your New Task

• Advising, accompanying and supporting rehabilitands during vocational training in the vocational training work
• Taxation and planning of rehabilitation processes in dialogical case management with focus on professional and social participation
• Enable access to resources and services and support in application
• Work in the multi-professional team and moderate reha plan conferences
• Documentation of educational and social work
• Report and vote with the cost carrier
• Recover and evaluate key figures

Your profile

• A completed study of social education (Bachelor Master or Diploma) or a comparable qualification
• Openness and sensitivity for the special situations of young people
• Communication strength and enforcement in use for participants
• Knowledge of social legislation in particular SGB II, SGB XII and care law
• Secure MS Office knowledge and experienced handling of EDV
• Experience in dealing with the target group of people with learning disabilities, mental impairment and/or autism spectrum
• Remarkable is the membership of a Christian church belonging to the working community of Christian Churches (ACK) in Germany
