Completed training for medical specialists (m/w/d), health care (m/w/d) or equivalent Training

Continuing training in case management or willingness to complete this Professional experience in case management desirable Very pronounced organizational and communication skills High sense of responsibility and commitment Pleasure to a trusted, interdisciplinary cooperation with all clinic areas as well as team skills Good computer skills

Your duties

Control of stationary planning and coordination of treatment processes Function as an interface between doctors, nurses, therapists:inside, medical controlling, social consulting as well as the central occupancy management Ensuring good interface cooperation and communication with the established doctors Ensuring case-related documentation and structural survey of case-relevant data Support and advice of doctors in the duration control Participation in early meetings and visits Care of process and quality instruments in medical and nursing areas (e.g. checklists, standards) Participation in process improvements Careful information and ensuring complete and correct documentation of ERAS patients Support of the care team as part of patient intake
