Nurse - Social Work and Patient Aftercare

At the Klinikum Nürnberg, Campus Nord, we are looking for a dedicated nursing specialist (m/w/d) for social work & patient care.

The position includes 19.25 hours of week and is limited until 31.12.2027. In this role, you will accompany patients after the stationary stay, assist in further care and actively contribute to a secure transfer to the outpatient care.

Tasks:

• Accompanying patients in redundancy and transition management
• Advice on supply and support services after the hospital stay
• Interdisciplinary cooperation with social services, care, doctors and doctors and relatives
• Documentation of all relevant services and transparent information flow
• Organization and coordination of treatments, rehabilitation and care
• Participation in a patient-centric and value-estimating aftercare design

Requirements:

• Senior Consultant (m/w/d)
• Social Service Experience, Case Management or Dismissal Management of Benefit
• Secure appearance, communication strength & joy in interdisciplinary cooperation
• High level of self-responsibility and structured working
• readiness to accompany patients and relatives empirically through transition situations
• Digital documentation is of course for you
