The patient managerin is the central contact personin and information providerin for the coordination and organisation of health and selected services The patient managerin organizes the reception planning of the stationary stay including necessary pre-stationary diagnostics He/she controls the hospital's resource capacities in the sense of optimal utilization He/She provides support for the collection of information (e.g. requesting external findings, doctor letters etc.) In addition, the tasks include: Determining aid needs, resources and risks for planning the assistance and support system (Assessment) Development and development of the internal and external network Contact care with cooperation partners, network partners Emphasis of patient care and support needs together with those affected and members of the multidisciplinary team (doctors, care, therapists, social services) Contact with other clinics after consultation with the medical service Coordination of laying and external clinics The patient manager*in works as part of a 5-day week from Monday to Friday in day service

Complete training and professional experience in a medical profession Experience from the area CM desirable Completed training for the certified case manager (DGCC) or the willingness to complete CM further education Comprehensive knowledge of coordination processes within patient flows Professional commitment and communication skills, enforcement skills Representative organisational skills and sense of responsibility, team skills and creativity, independent work Safe handling of the PC and the current MS Office product family Knowledge in the area of our hospital information system (Medico) desired Basic knowledge of the DRG system
