Independent information and advice on rights and services under the Social Code, in particular SGB XI and SGB XII Case Management and Care Consulting according to § 7a SGB XI Care Management: Coordination and further development of nursing and social care services Networking and coordination of nursing, domestic and social offers on site Public relations and documentation of activity Presentation, updating and cooperation in the further development of the offers Cooperation with cost and service providers, social services, self-help groups and other citizens

Graduate of Social Work or Social Education (w/m/d) with Diploma or Bachelor and State Recognition Additional qualification for care counselling according to § 7a SGB XI or the willingness to acquire it in a timely manner Multiannual professional experience in institutions/services in the field of counselling in the context of the elderly/care/medicine are desirable Knowledge of the methods for determining the individual need for assistance and/or experience in the generation, evaluation and interpretation of control data is advantageous Contact and team skills, method skills, strategic thinking skills, independent work and a safe occurrence Good knowledge of social performance law or the willingness to acquire these skills in no time Basic computer knowledge (Word, Excel), as well as knowledge in the field of computerized documentation systems or the willingness to acquire them in a short time Class B licence and the willingness to use the private car for reimbursement
