Tasks

• Planning and organisation of return to the social “reception room” (school, kita, training, etc.) at the end of the (partial)stationary treatment and further care in the outpatient setting
• Accompanying patients on site
• Consultation of all external references involved in the process
• Close cooperation with the (part-)stationary treatment teams and the school for sick people
• independent implementation of group offers
• Working in the care and education teams of the stations
• Permanent exchange with schools/school social workers in the region 10

Your profile

• Complete studies in social work with state recognition
• Professional experience in clinical social work
• A high level of flexibility, loadability, ready-to-use and team capability
• Structured and independent work
• empathy, organisational skills, communication and enforcement
• willingness to cooperate with the different stakeholders
