In Ev.-Luth. Kirchenkreis Hamburg-Ost are committed to people – in the church communities on site and in diverse areas of work that are summarized in the area of Diakonie+Education. Our two housing groups in Hummelsbüttel offer a total of 32 people with a mental disability. The majority visit a workshop for people with disabilities during the day. Our aim is to enable the residents to live as self-determined as possible and to participate in society. You support our residents in their way of life and promote their individual needs in the sense of comprehensive participation. You assist in basic nursing activities or act as deputy if necessary. You work closely with references, professionals and external institutions. You accompany our residents in all areas of life, such as leisure, household management or body hygiene. You organize and accompany excursions, holiday breaks and activities in the social area. You will receive and develop participation and participation processes. You create documentation, write social and progress reports and continue to write the 'individual assistance planning' (IHP). You contribute to the further development of our offers in order to enable the greatest possible participation. You participate in meetings, supervision and training.

You have completed training as health care and nursing caregivers, elderly caregivers, educators or nurses (m/w/d). You enjoy working with people with mental and physical impairment. You can work both in the team and independently and plan your action well. You occur value-estimatingly and empathically to residents, relatives, colleagues and external service providers. You bring good team, coordination and cooperation skills. You show commitment, sense of responsibility and social competence. You have good self-organization, are flexible and mobile and bring empathy. You are ready to work in shift and weekend service as well as in night readiness.
