Framework conditions: Place of use: Hannover, Hahne Residenz „Heidehaus“. Hours: full-time or part-time possible; the consulting function is 20 hours a week. Extension of hours: Additional weekly hours can be taken over as a nurse in the course of an activity. Working hours: consultations between 14:00 and 18:00; Date planning is done independently. Additional hours of care are provided in regular early service, late service and weekend service. Payment: For a weekly working time of 20 hours, the monthly gross salary is between € 2,100,18 and € 2,82,83, depending on the experience.

Your tasks: Consultation talks: conduct qualified consultations according to § 132g SGB V with residents and relatives. Precautionary planning: Advice on patient provision, precautionary power, care provision and other precautionary documents. Support & support: discussions on medical-care measures, palliative care and individual wishes during the last stage of life. Cooperation: Participation in multidisciplinary case meetings and coordination with internal and external contacts. Information exchange: implementation of short courses and information events. Quality development: participation in continuous improvement processes in health care planning.

Your profile: Qualification: Completed training as a nurse. Additional qualification: Health care planning for the last stage of life according to § 132g SGB V. Experience: At least one year experience in health care planning for the last phase of life according to § 132g SGB V. Working: Self-employed, structured and responsible. Communication: High empathy and professional handling of sensitive conversations and challenging situations. Organization: Good organizational ability and independent scheduling.
