Strengthen our outpatient care service in Lüneburg at the earliest possible time. The place is unlimited and in parttime with a weekly working time of 10-15 hours**.

Your tasks:

• Implementation of counselling talks in accordance with §37.3 SGB XI for nursing persons and their relatives in their home environment
• Transfer of appropriate care services according to the service complex catalogue
• Collection and analysis of the individual aid requirement taking into account the assessment of the medical service of the health insurance companies (MDK)
• Working together with the nursing staff and their relatives to develop individual care plans that are adapted to the personal needs and wishes and ensure the best possible quality of care
• advice, examination and support in the application or increase of care levels to ensure access to services
• Documentation and follow-up of consultations and agreed measures

Your profile:

• Complete training as a nursing specialist / nursing professional / nursing professional or health care professional Senior Caregiver
• At least 2 years of experience in outpatient care
• Continuing training as a nursing consultant according to § 7a SGB XI and deeper knowledge of the service complex catalogue of advantage
• Empathy and joy in dealing with people
• Representative communication and consulting skills
• Self-employed, structured and careful working
• Secure handling of MS Office, especially Word and basics in Excel
• Class B driving licence, ideally own car and regional readiness to travel
