You support our adult clients in the independent design and management of their everyday life and their leisure in their homes and families, in the acquisition of new skills and social participation. You're the first person-centred integrated participation plans (PiT), digital assistance plans, running and evaluating the documentation. You are in exchange with legal caregivers, relatives, institutions, therapists, doctors and connect in the social area. You plan your dates independently and in consultation with the team management and your colleagues.

You have a degree in social education, social work, medical education or a comparable qualification. You bring professional experience in working with people with cognitive impairment, are familiar with the BTHG, the Framework Contract 3 and the PIT. You are owned by a passenger car license and master the common PC applications.

Depending on the qualification and professional experience, you earn € 4.302 or € 4.645 including SuE allowance, for many years of professional experience up to € 5.486 are possible (based on TVöD-B pay group S12, full-time, part-time). There is also a special annual payment and an occupational pension scheme. With the “Vitos Card” you get a pay extra of up to 50 € per month; tax- and social insurance-free for shopping, tanking, supermarkets or in the restaurant. We support your professional and personal development by working intensively, including professional guidance and regular team meetings, case meetings and supervision.
