Registered Nurse / Team Leader Outpatient Care (m/f/d)

For our outpatient tour care in Freital, we are looking for a nursery with interest in team management (m/w/d) at the next possible time. The place is to be occupied in part-time (30-35 hours/week).

Your duties

• care organisation (organization of workflows in the team and individual work organisation); handovers, case meetings etc.) after consultation with the care service

• Cooperation and coordination tasks (e.g. with external partners, doctors, relatives)

• Individual and needs-oriented care of our clients

• Tour planning

• Implementation of consultancy visits pursuant to § 37 para. 3 SGB XI

• Review of professional documentation

This will bring you

• Completed three-year training in nursing or nursing

• German B2- Level

• Driving licence Kl. B

• Very good communication skills
