Employee in the field of rehabilitation specialist with assumption of case management tasks (m/f/d)

We are looking for an employeein the Reha service for the function as a case managerin.

The position is limited to one year and must be filled in full time.

Your tasks

With regard to professional and social integration into society, you as a case manager*in the Reha specialist service, promote the social participation of young people with mental impairments.

Process control: As the main contact partner*in close coordination with the cost providers, you take over the control of the entire rehabilitation process for the participants.

Consultation & promotion: The expert advice is one of your tasks as well as the targeted promotion of the personal and professional development of the participants.

Cooperation: In close cooperation with training, housing and accompanying specialist services, a holistic support of young people is ensured.

Document & Communication: You use your good PC skills routined, provide a structured and sustainable documentation of the reha processes and introduce your strong communication and consulting skills in everyday work.

Continue development: They actively participate in the further development of processes. In addition, you will have a high level of readiness for continuous further education and training.

Your profile

Qualifications: You have a degree in social education, social work, pedagogy or special education with the degree Diploma, Bachelor or Master.
