Organisation and mediation of outpatient and inpatient care services, e.g. cared for housing, care services, self-help groups and care facilities Application and coordination of services for sickness and nursing pensions and other costs Advice on social legislation, in particular on health and care insurance, the right of disabled persons and the right of care and the right of care Support for social reintegration, introduction of rehabilitation measures and long-term therapies, as well as securing the financial/tangible basis of life Establishment and maintenance of regional supply networks and close cooperation with external institutions Active participation in structured dismissal management, participation in visits and multi-professional team meetings Independent implementation of social therapy

Complete study of social work or social education (diploma or Bachelor) Multiannual professional experience in psychosocial work, ideally with people with mental illnesses or addiction diseases High motivation for work in the psychiatric environment Knowledge of social law and relevant social legislation (e.g. SGB) Self-employed, structured and responsible work Impressed communication strength and team capacity Cooperation and interdisciplinary thinking in the multi-professional team
