Educator / Social worker - Outpatient assistance in one's own home (SGB IX) (m/f/d)

For the seeking assistance in our own living room according to SGB IX, we are looking for a dedicated specialist who accompanies and supports our clients on their way to an independent life.

Join us in part-time (30 hours/week) and become part of a meaningful activity with a lot of self-responsibility from 01.10.2026.

Your tasks

**You develop individual support offerings based on the needs of your clients. As part of qualified assistance, you accompany people with different support needs in everyday life, promote their self-employment and strengthen their existing resources.

Mobility: The seeking assistance takes place in the living environment of your clients. You plan your dates independently and visit them in your own living room. You can use your own car for the paths between the places of use (the kilometers you traveled will of course be refunded). Your office location is in Bad Bramstedt.

Network work & documentation:You work closely and reliably with relatives, authorities and other institutions. You participate in help schedule discussions and ensure a structured documentation as well as the preparation of development reports.

Your profile

Qualifications: You are an educatorin with relevant additional qualifications or social educationin (alternative medical educationin, ergotherapeutin or cf.). In addition, you have knowledge of systemic work and could ideally gain experience with the ICF.

Engagement: You own the class B license and your own car. Your working hours, mainly in the afternoon hours, are your own responsibility and as required. Knowledge of how to use MS Office completes your profile.

Competition: Your clients are a facing contact person. With the strength of communication and empathy, you shape cooperation and keep the well-being as well as the target agreements always professionally and transparent in view.
