In the project “Wohnen stay in the district according to § 123 SGB XI”, we are currently developing new, innovative supply approaches for people in the district of Hamburg Bergedorf. The goal is to create sustainable support structures and successfully implement Community Health Nursing. As a Community Health Nurse, you will be there from the outset, take a central role in primary care and shape the further development of the Profession Pflege. We are currently building a new team on Bachelor level (1 CHN, 1 CHN in management function as well as 1 person with an educational qualification e.g. Bachelor/Master Social Work). You think care for different target groups such as seniors, families, people with chronic illnesses and people with disabilities, socially oriented and conceptually, and bring your care expertise – ideally with CHN/high school qualification – into prevention, education, assessments as well as case/care management and network coordination. Their focus is on location in the Bergedorf region. Office facilities are available at various locations. Above all, you are looking and active in the quarter; Mobile work is possible in proportion. Together with us, you develop standards and processes and improve primary care sustainably. The project is scientifically supported by a university in the context of role development. The occupation of posts is subject to a financing requirement of the authorities.

Development and implementation of innovative concepts in the field of “Community Health Nursing” Design of highly complex care processes on a scientific and practical basis Cooperation with research and quality development projects Cooperation with stadium-related, regional and interdisciplinary teams and networks to promote preventive and cross-sector supply Close exchange with clients, implementation of assessments and creation of individual supply plans Support and mentoring for professionals, specialist services and also relatives mediation of health competence through client-oriented training to accompany complex health situations Continuous optimization and integration of assessments into supply planning Development of sustainable processes for corporate implementation after project completion Scientific work as a Community Health Nurse e.g. in the form of publications and presentation of results at congresses

Conceptual thinkers with heart blood and deed for care! Motivated nursing experts with completed studies in the field of nursing, preferably community health nursing, advanced practice nursing or comparable courses such as nursing, nursing or public health As an alternative, nursing staff with a completed training as a health care and nursing staffin, nursing staff / nursing staff, nursing staffin or a similar professional qualification with well-founded professional experiences that are in study or have an interest in one of these courses Ideally knowledge of the forms of care (age care, outpatient care, day care or the like) The ability to quickly capture actual situations, to develop innovative solutions and to drive them forward with a high level of implementation Experiences in the care of various target groups such as people with chronic diseases, people with disabilities, families or seniors are desirable.
