Your New Task

• Coordination, guidance and documentation of the rehabilitation process for young people with learning difficulties and mental illnesses
• Cooperation in the interdisciplinary rehabilitation team
• Organisation and implementation of team and rehabilitation conferences
• Compilation of technical reports for the cost carrier
• Networking with cooperation partners (m/w/d)
• Ensuring the integration success of participants

Your profile

(m/w/d) with further education on the case manager or completed university studies of social education, social work or comparable qualification

• At least two years of professional experience in the field would be desirable
• Team skills as well as structured and independent work as an organizational talent
• Knowledge of the current training and labour market would be of advantage
• Documentation security and safe handling in MS-Office
• Remarkable is the membership of a Christian church belonging to the working community of Christian Churches (ACK) in Germany
