Your tasks:

• Plan your leave management – you organize care, help in everyday life, living/working perspectives and transitions.
• Networks link – you coordinate cooperation with authorities, cost-beneficiaries, support, ambulances and help.
• Crises accompany – you conduct stabilisation/motivation talks, involve relatives and moderate fall conferences.
• Save documentation – you document structured, start reports and participate in quality processes.

Qualifications

You complete our team if you...

• a completed study of social work/social education.
• have professional experience in psychosocial care and ideally employs knowledge of social legislation (SGB II/XII).
• work flexibly, empathically and structured and likes to work in the team.
• Self-initiative shows, communicative and personally strengthened.
