The city of Bremerhaven (approximately 120,000 inhabitants:inside) is looking for a:n for the social medical service for adults of the Bremerhaven Health Office at the earliest possible time.

S 11b TVöD (deficit order/VKA: 3.915,12€ to 5.440,57€/full time) -

for the duration of a parental representation.

The Department of the Social Medical Service for Adults has the task of providing and supporting adult citizens on the basis of social legislation in the management of an independent life in the health sector.

The area of responsibility includes:

• Consultation and assistance in problems and conflicts
• Advice and communication of assistance
• Opinions on necessary assistance and appropriate advice
• Ensuring a need-oriented care for help and care-needed citizens:
• Cooperation with established doctors: indoors, hospitals, offices and institutions, outpatient care services, old and nursing homes, self-help groups, etc.
• Documentation and correspondence.

A prerequisite for an application is:

• a completed study of the Diplom-Socialpädagogik/Diplom-Socialarbeit (FH) with state recognition
• the completion of the Social Work Programme B.A. with state recognition or
• the completion of a comparable social education course.

Such professional experience is desirable.
