Your tasks: Responsibility for targeted and systematic organization over the entire course of treatment and project in case/case management for children and young adults with rare diseases Holistic clinical process control: coordination and monitoring of the entire course of treatment – from resource-oriented reception and occupancy planning to retention and redemption Structured Assessment & Treatment Planning: Responsibility for a targeted nursing, medical and psychosocial assessment including diagnostics, documentation in the KIS and cross-sectoral demand survey Effective discharge management: Control of a complete connection supply according to § 39 para. 1a SGB V – including delegation, social security clarification and close coordination with internal and external supply structures

Completed 3-year vocational training in nursing in the areas of health and (children)care, nursing, medical professionals or similar qualifications, and ideally DGCC-certifiedr Case Managerin or willingness to continue Founded clinical experience: Multiannual professional experience in the hospital with a strong nursing expertise and a secure understanding of clinical organizational structures Communicative and digital strength: Representative consulting and conversation management skills combined with secure digital skills pronounced resilience and the ability to independently find constructive solutions even in very challenging situations
