Structured post-care planning: Together with the treatment team, ensure smooth dismissal planning and post-stationary care of patients, create help plans and develop meaningful reporting for your area of responsibility. Full social consultation: In the clinical social service, you advise and accompany patients, patients and relatives on social, psychosocial and social issues. Networked assistance coordination: You support patients in disease management, provide appropriate assistance services and coordinate cooperation with external institutions, authorities, cost centers and other network partners.

You bring a completed study of social pedagogy or social work (BA/FH/Uni) and have already gained first leadership experience in dismissal management or a comparable area. Very good knowledge of the relevant legal foundations, in particular in social law, care law, serious disability law, foreign law as well as unemployment and social assistance law, distinguish your professional profile. The safe handling of MS Office is of course for you; ideally you also have knowledge of the ORBIS system. Organisational skills, coordination skills and a performance- and responsible way of working.
