The clinic for psychiatry, psychosomatics and psychotherapy at the Centre for Mental Health has 7 open psychiatric stations, 2 protected stations (between 18 and 21 beds) and 3 day clinics. Treatment focuses on bipolar disorders, severe depression, dependency disorders, Gerontopsychiatrie, schizophrenic psychosis. The Kliniksozialdienst takes over the dismissal planning of the patients as part of the dismissal management according to § 39 SGB V, in cooperation with a multi-professional team. This includes the planning and coordination of dismissal, taking into account the health and social, professional and economic situation. In addition to the medical and nursing treatment of patients, social work thus makes an important contribution in the overall dismissal concept of treatment. The position is to be occupied at the clinic for psychiatry, psychosomatics and psychotherapy as soon as possible. Your tasks: Consultation and support of patients and their relatives in social matters and ensuring post-stationary care. Advice and support for medical and professional rehabilitation measures. Advising and starting up the post-stationary perspectives. Interdisciplinary cooperation of all professional groups. Cost clarifications with institutions and institutions. Contact design with the institutions involved, e.g. social offices or employment agency. Structure, participation and moderation in group work. You have a completed study of social work or social education with state recognition. You possess sound social knowledge and a high degree of consulting competence.
