Participation in early and case meetings for reception and reception planning and patient visas Coordination of patient assignment and the entire treatment process in close coordination with the medical service, care and other professional groups Further development and coordination of the release management according to § 39 SGB V Ensuring complete further care by incorporating outpatient and outpatient structures across sector boundaries Participation in the optimization of processes and workflows and the promotion of constructive interdisciplinary cooperation Communication and coordination with external bodies to ensure therapy / insurance status Interface function of all cross-sector players

Completed medical or commercial training as, for example, health and nursing staff, OTA (m/w/d), MFA (m/w/d), merchant/w/d in healthcare or comparable qualification Good knowledge of escape management Professional experience of at least two years desirable Very good knowledge of German (min. C1) and good knowledge of English (min.
