Case Manager Outpatient Intensive Care (m/f/d)

You control new supplies from the first request to the stable day of care at home. You are organising the transfer from clinic, ventilation center or (children) intensive care unit to domestic care. You're conducting first interviews with patients, relatives and treatment teams. You agree with cost-beneficiaries on regulations, permits and subsequent requests You co-ordinate doctors, respiration centers, SPZ, therapists, helpers and sanity houses as part of the transfer You are planning and building cooperations with clinics, social services and instructors You are a problem solver for clinics and relatives and always work patient-centered

Your professional experience, both in the social service or in the case management of a hospital and in the extraclinical intensive care, makes you an ideal candidate or ideal candidate for this position You can look back on a completed training as a nursing specialist and/or have completed a study in health care, such as in social work or care management Your further training to the Case Manager (DGCC) will also qualify you for the challenges of this role Very good German skills in word and writing characterize you and are essential for communication in your working day You quickly build relationships with people, spray with deeds and have an extraordinary work morale
